Declaration (please read carefully)

I hereby apply for membership of Moore Park Golf Club Limited, and if elected, I agree to be bound by all the provisions of the club’s constitution and I further request that my name be placed on the register or members in the appropriate category. I agree that if I elect to pay my annual subscription by installments either in the first year of membership or subsequent years and I resign before the end of the membership year in question that I will be liable to pay the equivalent of three months membership. I also agree that if I elect to pay my joining fee by installments and I resign before the joining fee has been fully paid that I will be liable to pay the full remaining outstanding balance.
